Logging request headers to customdimensions

Alexandre Vincent requested to merge avincent/request_headers_to_appinsights_customdimensions into master

Here we log some request headers for the purpose of making them visible on azure appinsights and usable in azure workbooks. This is to gather metrics about "how" the service is used.
