Document the extra header fields for websocket handshake

Compared to strict ETP1.2 standard, data-partitionid extra header field is required for websocket handshake. It is not enough documented. Please document it in partition of the root README. Also document the field client-id in the source code : it is optional and allow more verbosity in server response about the related HTTP session.
