Current Behavior or Linked Issues

Added a template for the issues. Issues are being used to track the work. This will help the ticket raiser to fill in all details that the assignee expects.

This can be a starting point and we can improve.

NOTE: There might not be a clean feature by GitLab to provide templates to Issues. But we can enforce this by the process.
